Housing operators

Help us understand
your home.

Tell us who you serve, what your home offers, and which residents may be a good fit.

Our operator intake collects details about the property, room types, rates, funding sources, services, house rules, and admission considerations.

Please submit one property per form.

What we review together

  • Actual housing or program type
  • Populations served and admission requirements
  • Room types, current openings, and rates
  • Services, support levels, and house rules
  • Licensing or program information, when applicable

Partnership terms are discussed individually based on the housing or program category and the referral arrangement.

Referral & community partners

A connection for
your client’s next step.

We welcome connections with case managers, social workers, discharge planners, shelters, treatment providers, and community organizations.

Reach out about an adult’s housing needs or to introduce your organization. We’ll clarify what information is needed to begin reviewing potential options.

Useful starting details

Share the housing type sought, preferred area, approximate budget or funding source, timeframe, and general support needs.

Please coordinate any sharing of personal information with the client and your organization’s process.

Community & Resource Partners

Support for the steps
beyond housing.

A place to live is one part of getting back on your feet. Access to food, employment, transportation, and other everyday resources can matter, too.

We welcome relationships with organizations that provide practical support for adults and families. Introduce your services, who you serve, your service area, and how people can reach you.

As our network grows, we aim to help people explore outside resources that may fit their needs. Each organization provides its own services and determines eligibility and availability.

Organizations we welcome

  • Employment and workforce development programs
  • Food banks and meal assistance programs
  • Transportation resources
  • Shelters and community outreach organizations
  • Other organizations offering practical support

We welcome new connections. Resource referrals depend on available relationships, program requirements, and each person’s needs.

Placement & home care partnerships

Housing connections today.
More support ahead.

Kindred Pathways helps operators connect with prospective residents whose housing needs may fit their homes.

As our non-medical home care agency develops, we look forward to discussing additional support for residents who may benefit from personal care, companionship, or help with everyday activities.

Home care coming soon

Two services. Individually arranged.

Future home care services will be arranged and billed separately from placement and referral services, with attention to each resident’s needs and the home’s existing services.

Operators are welcome to contact us about future collaboration. Service availability and payment options will be announced at launch. Additional payer options may become available as required enrollment, credentialing, and approvals are completed.

A shared purpose

Strong relationships support
more informed next steps.

Our focus is understanding each person’s needs and what a home, program, or community organization actually offers before suggesting a potential connection. Clear information helps people make more thoughtful decisions about housing and outside support.
